4. Address of the property you wish to lodge a complaint against

It is important to provide as much detail as possible regarding the complaint or violation (ie. nature and description of complaint, description of the violation, date of the violation, etc.). Lack of information may result in delays in response time and resolution.

Personal information requested on this form is collected under the authority of section 11 of the  Municipal Freedom of Information and Protection of Privacy Act, R.S.O. 1990, c. M.56 (Ontario)., as amended. The personal information you have provided will be used to administer and contact you regarding the above complaint. The Town of Whitchurch-Stouffville will only disclose your personal information to staff who require the information to perform the investigation.
