Police Record Check

The Whitchurch-Stouffville Museum & Community Centre, as part of the Corporation of the Town of Stouffville, recognizes the need to protect its vulnerable citizens, as well as the Museum's assets. Therefore, the Museum & Community Centre will use Police Record Checks as a screening tool where appropriate.

Volunteer applicants will be notified prior to selection if a Police Record check is required. If required, the results of the police record must be provided prior to finalization of the selection; failure to do so will result in the candidate being declined.

Personal information requested on this form is collected under the authority of section 11 of the  Municipal Freedom of Information and Protection of Privacy Act, R.S.O. 1990, c. M.56 (Ontario)., as amended and will be used to determine eligibility and suitability for volunteer work with the Whitchurch-Stouffville Museum & Community Centre.
