*** Please Note:  Your Landlord/Property Owner must complete and submit the Landlord/Property Owner Declaration form prior to you submitting this request. 

If the Landlord/Property Owner Declaration form is not submitted, your application for Property Tax Rebate for Registered Charities will only be held for 30 days.

This form must be fully completed for your application to be considered.  

The deadline for submitting this application is February 28 of the calendar year following the year which you are requesting a charitable rebate.
